Staff Forms

We are so excited to have you on the team this summer! Please read this information carefully and make sure you complete everything before you arrive at Camp!

Health Form

An annual physical completed within 12 months prior to your arrival and signed by your physician is a condition of employment and must be received at Camp prior to your arrival. Student health services at your school, county, and city health departments, and walk-in clinics are great options if you have trouble getting an appointment with your physician. 

Please print your Health Form and take it with you to your appointment.

Youth Employment Certificate 

This is only for new staff members who are under the age of 18. Staff members under the age of 18 are required to complete a North Carolina Worker's Permit. This is required for all in-state and out-of-state employees who will be under the age of 18 for any portion of their employment at Camp. Please read these instructions before doing so. The form will ask you some questions and the answers are in this document! 

Redwoods Institute Online Training

Your Redwoods training must be completed prior to arrival and is required for all new and returning staff members. These trainings remind us of the importance of protecting ourselves while working with children. Click on the appropriate instructions below if you're a new or returning staff member. 

RETURNING Staff Redwoods Instructions

I-9

(Only for new staff and anyone who last worked at Camp in 2015)

When you check in, new staff will be required to complete the Employment Eligibility Verification Form I-9, which is a U.S. Citizenship and Immigration Services form. It is used to verify an employee's identity and to establish that the employee is eligible to accept employment in the U.S. You will be asked to show identification. If you choose a document from List A (such as your passport), you will only need to bring that one document. If you do not choose a document from List A, you will need to bring a document from List B and a document from List C. You MUST bring the original documents with you to Camp at check-in; we cannot accept scanned versions.

Onboarding

You should receive an email soon from the YMCA of the Triangle to complete your onboarding. You have to complete your onboarding within one week of receiving this email, so be on the lookout for it! 

Transportation Information

If you are planning on flying to or from Camp, please review our travel guidelines to help as you are making your travel plans. This guide gives specifics on when to arrive, what airlines to use, what airports to fly into, etc. Please review our Travel Guidelines before you make your travel plans. For those flying to or from Camp, please fill out our Travel Questionnaire so we know when to pick you up from the airport! Everyone flying to or from Camp must fill out this survey.

We'll be in touch as we get closer to the summer with more information about medications, time-off requests, special time-off changes, age group preferences, and payroll set-up. Make sure you've completed everything above and scan/send completed documents to Gretchen Bohman at Seafarer or Dannett Boyd at Sea Gull. 

Checklist of things to do BEFORE you arrive at Camp!:

  • New staff and anyone who last worked in 2015 bring I-9 documents with you to Camp
  • New staff under 18 complete your workers permit
  • All staff complete a health form (and get a physical with your physician)
  • All staff complete onboarding
  • All staff complete Redwoods training
  • Complete the Travel Questionnaire if you're flying to or from Camp!
